I thought a lot about death during this period. I firmly believed in the afterlife and eternal salvation. I knew that Barbara was a good Christian and that her place in heaven would be a certainty. But I wanted her here on earth with me for the rest of my life. I wanted to see her grow old and comfort me. I couldn't stand the thought of her dying before me.

In 1967, an Italian surgeon touring Amsterdam's Rijks museum stopped in front of Rembrandt's Bathsheba at Her Bath, on loan from the Louvre, and noticed an asymmetry to Bathsheba's left breast; it seemed distended, swollen near the armpit, discolored, and marked with a distinctive pitting. With a little research, the physician learned that Rembrandt's model, his mistress Hendrickje Stoffels, later died after a long illness, and he conjectured in a celebrated article for an Italian medical journal that the cause of her death was almost certainly breast cancer.

A horror known to every culture in every age, breast cancer has been responsible for the deaths of 25 million womenthroughout history. An Egyptian physician writing 3,500 years ago concluded that there was no treatment for the disease. Later surgeons recommended excising the tumor or, in extreme cases, the entire breast. This was the treatment advocated by the court physician to sixth-century Byzantine empress Theodora, the wife of Justinian, though she chose to die in pain rather than lose her breast. Only in the past few decades has treatment advanced beyond disfiguring surgery.

In Bathsheba's Breast, historian James S. Olson -- who lost his left hand and forearm to cancer while writing this book -- provides an absorbing and often frightening narrative history of breast cancer told through the heroic stories of women who have confronted the disease, from Theodora to Anne of Austria, Louis XIV's mother, who confronted "nun's disease" by perfecting the art of dying well, to Dr. Jerri Nielson, who was dramatically evacuated from the South Pole in 1999 after performing a biopsy on her own breast and self-administering chemotherapy. Olson explores every facet of the disease: medicine's evolving understanding of its pathology and treatment options; its cultural significance; the political and economic logic that has dictated the terms of a war on a "woman's disease"; and the rise of patient activism. Olson concludes that, although it has not yet been conquered, breast cancer is no longer the story of individual women struggling alone against a mysterious and deadly foe.

How perceptions of breast cancer and its treatment have evolved over the centuries, particularly since the advent of the women's movement. In what he calls "self-administered psychotherapy" following the loss of his left arm to an epithelial sarcoma, Olson (History/Sam Houston State Univ.) immersed himself in the study of breast cancer's history. Not only is there a rich body of literature by prominent victims, but the gender dynamics of the disease—invariably female patients treated by generally male doctors—provide a fascinating view of the ways in which culture, politics, and science interact. Stories of individual women's battles with cancer, ranging from Persian Queen Atossa in the fifth century b.c. to American scientist Jerri Nielson in 20th-century Antarctica, provide a dramatic introduction. Olson then details how views of the disease have altered since the time of Hippocrates, who believed its cause was an excess of black bile in the body, and how treatments have evolved partly as a result of scientific advances and partly through cultural changes. While pre-anesthesia mastectomies seem downright barbaric, the widespread performance in the 1950s of mutilating and debilitating super-radical mastectomies followed by further surgery to remove the ovaries, the pituitary, and the adrenal glands, is no less repulsive. Today, Olson reports, such treatments have been supplanted by far less drastic surgery, radiation, and chemotherapy. He recounts how the women's movement, the pop-culture obsession with breasts, the willingness of some physicians to test alternatives to radical mastectomy, and the development of mammography permitting earlier diagnosis have changed the optionsavailable to women with breast cancer. He describes how activists have moved from providing emotional support to breast cancer patients to challenging the medical establishment and lobbying the government for research money and legislative changes affecting insurance coverage. Its stigma disappearing, breast cancer has now moved onto book and magazine covers and into sitcoms. A lucid account of an ongoing war on a changing battlefield with at least the hope of new weapons.

In this first reference book on autistic spectrum disorders (ASD), Neisworth and Wolfe (both special education, Pennsylvania State Univ.) have produced a much-needed resource on terms associated with this disorder. Around 500 entries-ranging from one to three paragraphs-cover topics like diagnoses, therapies, and associated disorders, with numerous cross references. The authors have written the entries in a clear style that makes understandable even the most complicated concepts associated with ASD. For example, the hard-to-grasp concept of mindblindness (also known as theory of mind) is defined as the "difficulty in sensing and appropriately reacting to the beliefs, desires, emotions, and intentions of others." While fairly comprehensive in its coverage, the encyclopedia would have been improved by including profiles of prominent people in the ASD field, including Temple Grandin and Dr. Bernard Rimland. The entries also lack specific references and web sites that would have made the information more useful, though two appendixes list assessment tools and organizations. Bottom Line Despite its minor shortcomings, this is a welcome addition to the literature on ASD that will hopefully be expanded in subsequent editions. Purpose: GERD is the most common reason patients are referred for upper endoscopy. It affects one-third of Americans with some regularity and this book is aimed at patients and the public to dispel some of the myths regarding this disease. It provides down-to-earth information about causes and current treatment of heartburn. These are certainly worthy objectives and the authors succeed admirably in making the information understandable and palatable.
Audience: The book is aimed at GERD patients and their families.
Features: Different sections address definitions and facts; symptoms and atypical manifestations of GERD; hiatal hernias, the lower esophageal sphincter, and reflux; lifestyle, medications, diet and GERD; complications of GERD; doctor's evaluation; testing for GERD, H. pylori, GERD, and peptic ulcer disease; medications for GERD; and endoscopic and surgical options for GERD. Current information is provided in a common sense manner on the relationship between hiatal hernia and GERD, lifestyle issues, the impact of diet modification, and the importance of the identification of Barrett's esophagus as a precursor to esophageal cancer. The pharmacologic information as well as insights into surgical and endoscopic techniques are superb. The illustrations are also appropriate and informative. Iparticularly appreciated the comments by a GERD patient on his particular experience.
Assessment: This is a lovely informative addition to the 100 Questions and Answers series. GERD patients and families will find a great deal of useful information. Frankly, I can think of no other questions that these patients have asked me over the years. I will definitely refer my patients to this Lahey Clinic guide.

This book draws on the latest medical research to shatter some of the biggest myths about aging, and to outline a new way to understand and manage the aging process.

Old age gets a bad rap, argues geriatric specialist Lipschitz. The senior years do not inevitably mean loss of mental or physical prowess, he maintains: in fact, the cliched advice to "take it easy, you're not as young as you were" is exactly the wrong approach. Lipschitz cites studies showing that exercise is beneficial to the elderly, and that sexually active seniors live longer than celibate ones. He even decries the obsession to lose weight, claiming that "thin isn't healthy," since "pleasingly plump" (but not obese) people live longer. Lipschitz's approach is balanced and well reasoned, never fanatical. He argues against invasive surgery for most coronary disease, contending that if a person has no symptoms, surgery can cause more problems than it solves. However, he goes on to list specific conditions that do warrant surgical intervention. Many seniors are over-medicated, he notes, which leads to negative side effects, and he offers specific advice for replacing dozens of medications with a few carefully chosen ones. The host of a public television series called Aging Successfully with Doctor David, Lipschitz is articulate and entertaining, and he clearly explains medical terms and procedures for the layman. Personal trainer and yoga instructor Urla has created (and trademarked) Yogilates, a fitness regimen that combines calming Hatha yoga with Pilates, a body alignment system developed by deceased posture guru Joseph H. Pilates. If the combo seems a bit complicated, it is, and Urla's how-to manual doesn't make it easier to understand. Lessons such as "how to establish the pelvis in a neutral position" or "how to lift the legs into you using the core muscles" might be manageable if it weren't for their hard-to-follow instructions. The remaining third of the book is devoted to advanced Yogilates. As in previous sections, Urla as well as chiseled yoga and Pilates teachers grimly illustrate the positions. But even Urla warns "not to proceed in haste." He also urges, in addition to thrice-weekly Yogilates sessions, aerobics three to five days a week. Ultimately, trying to follow Urla's fitness program is the real workout-and may inspire readers to unfurl their yoga mats just to relax. b&w photos.
